This will let me know it’s your first time seeing me and allow us to discuss your treatment.Please download and fill out both forms before your appointment; Case History Form & Treatment Consent Form or arrive 10 minutes early to fill forms out in person.
Prenatal Clients:
Please select one of the ‘Prenatal Massage’ options during booking, this notifies me in my schedule and allows me to setup just for you!
